Informeret samtykke til psykologbehandling hos We.Care
Før du påbegynder behandling, skal du give dit informerede samtykke til at modtage psykologbehandling samt til, at We.Care må behandle dine personoplysninger, herunder helbredsoplysninger, i den forbindelse.
1. HVAD INDEBÆRER BHANDLINGEN?
Behandlingen består af psykologfaglige samtaler via video eller chat med en autoriseret psykolog eller psykolog med praksisuddannelse (klinisk psykolog).
Formålet er at:
- Undersøge og afdække dine problemstillinger
- Iværksætte relevant psykologisk behandling
- Vurdere behov for ændring eller afslutning af forløb
Behandlingsmetode og tilrettelæggelse beror på psykologens faglige vurdering.
2. FRIVILLIGHED
Det er frivilligt at modtage behandling.
Du kan til enhver tid:
- Afbryde behandlingen
- Undlade at besvare spørgsmål
- Trække dit samtykke tilbage
Tilbagetrækning påvirker ikke lovligheden af den behandling, der allerede er sket.
3. JOURNALFØRING
Psykologen har pligt til at føre journal i henhold til gældende sundhedslovgivning, herunder autorisationsloven og regler om journalføring.
Journalen indeholder relevante oplysninger om:
- Din problemstilling
- Behandlingsforløb
- Faglige vurderinger
Journalen opbevares i mindst 5 år efter sidste notat.
4. TAVSHEDSPLIGT
Psykologen er underlagt tavshedspligt.
Oplysninger videregives kun:
- Med dit samtykke
- Hvis lovgivningen kræver det
- Hvis det er nødvendigt for at afværge alvorlig risiko for skade på dig eller andre
Enhver videregivelse begrænses til det nødvendige.
5. FORSIKRINGSFORLØB ELLER ANDEN BETALER AF DIT FORLØB (HVIS RELEVANT)
Hvis dit forløb betales af en forsikring eller anden betaler, vil eventuel videregivelse af helbredsoplysninger kun ske på baggrund af særskilt samtykke.
6. DINE RETTIGHEDER
Du har ret til indsigt i og berigtigelse af dine oplysninger i overensstemmelse med gældende databeskyttelseslovgivning. For en fuldstændig beskrivelse af dine rettigheder og vores behandling af personoplysninger, herunder retten til dataportabilitet, indsigelse og klage til Datatilsynet, henvises til vores privatlivspolitik.
7. BEKRÆFTELSE
Ved at acceptere bekræfter du, at:
- Du har modtaget tilstrækkelig information
- Du har haft mulighed for at stille spørgsmål
- Du giver samtykke til at modtage psykologbehandling
- Du giver samtykke til behandling af dine personoplysninger, herunder helbredsoplysninger

Informed Consent to Psychological Treatment at We.Care
Before commencing treatment, you must provide your informed consent to receive psychological treatment and to allow We.Care to process your personal data, including health data, in connection therewith.
1. WHAT DOES THE TREATMENT INVOLVE?
The treatment consists of professional psychological consultations via video or chat with an authorized psychologist or a psychologist with clinical practical training (clinical psychologist).
The purpose is to:
- Investigate and assess your psychological challenges and concerns
- Initiate relevant psychological treatment
- Assess the need for adjustments to or termination of the course of treatment
The treatment method and planning are based on the psychologist’s professional assessment.
2. VOLUNTARINESS
Receiving treatment is entirely voluntary.
You can at any time:
- Discontinue the treatment
- Refrain from answering questions
- Withdraw your consent
Withdrawal of consent does not affect the lawfulness of the treatment that has already taken place prior to the withdrawal.
3. RECORD-KEEPING (JOURNALING)
The psychologist is statutory bound to maintain a clinical journal in accordance with applicable health legislation, including the Danish Act on Authorisation of Healthcare Professionals (autorisationsloven) and regulations regarding medical record-keeping.
The journal contains relevant information regarding:
- Your psychological challenges and concerns
- The course of treatment
- Professional clinical assessments
The journal is stored for a minimum of 5 years after the date of the final entry.
4. PROFESSIONAL SECRECY AND CONFIDENTIALITY
The psychologist is bound by professional secrecy and confidentiality.
Information is only disclosed to third parties:
- With your explicit consent
- If required by law
- If necessary to avert an acute and serious risk of harm to yourself or others
Any such disclosure will strictly be limited to what is necessary.
5. INSURANCE COURSES OR OTHER THIRD-PARTY PAYERS OD YOUR COURSE (IF APPLICABLE)
If your course is funded by an insurance company or another third-party payer, any disclosure of health data will only occur on the basis of a separate and specific consent.
6. YOUR RIGHTS
You have the right to access and rectify your personal data in accordance with applicable data protection legislation.
For a complete description of your rights and our processing of personal data, including the right to data portability, the right to object, and the right to lodge a complaint with the Danish Data Protection Agency (Datatilsynet), reference is made to our Privacy Policy.
7. CONFIRMATION AND DECLARATION
By accepting, you confirm that:
- You have received sufficient information regarding the treatment
- You have had the opportunity to ask questions
- You grant your consent to receive psychological treatment
- You grant your consent to the processing of your personal data, including health data
The counseling and guidance consists of professional consultations and support via video, telephone, or chat with a qualified consultant/advisor.
The purpose of the course is to:
- Assess your current issues and need for guidance.
- Provide you with concrete tools, support, and professional perspectives to manage your challenges.
- Note: Counseling and guidance is not to be equated with medical or psychotherapeutic treatment (treatment of mental illness) but is a supportive and advisory service.
